If you have scheduled a Naturopathic, Nutrition, Breast, or  Holistic Cancer Consultation …..

These forms will be emailed to you by the office, so please look for them in your email (check your spam folder as well).  Please have those forms completed when you arrive for your appointment.  If they are not completed, the office may have to reschedule your visit for another day, as they make take up a large portion of your visit time.
